At this time, Child Find screenings are being held virtually unless a face-to-face screening is requested when speaking with the office.
Step 1: Complete the Child Find Referral online, by phone, or by fax.

Step 2: You will be contacted by the Child Find office to have an appointment scheduled with a Child Find Specialist.
Step 3: Please fill in these forms and email them back to your Child Find Specialist before your appointment.
